User Forms


 
The Coordinator Form is used to request or revoke coordinator access to a facility. It must be filled out according to the instructions inside the form and then submitted to the FEDdesk Helpdesk {feddesk.helpdesk@gsa.gov} via a manager's Government email account. The email will serve as the official authorization of the request. Please do not alter the format of the form or its document type.

The ETAMS USER Form is provided for our customers' use. It can be used to grant/revoke/track ETAMS access within their own facility.
{Note: This form is submitted and approved within the client's organization. Please do not send to the helpdesk.}

The WEBPACC Form is to request access to the National Payroll Branch's Web PAR Accounting Code Changes application. Instructions for completing and submitting the form are found on the form.
